Fat Transfer Breast Augmentation
Breasts play a key role in the attractiveness and sensuality of the female body. Mother Nature has endowed girls and women very differently, and many long for slightly fuller breasts without wanting a drastic change. When breast implants are not an acceptable option, or when we aim for a moderate enlargement, we recommend fat transfer breast augmentation. The procedure is also suitable for men who would like a more defined chest.
Before fat transfer to the breasts, we first harvest your own fat via liposuction, which also allows us to contour areas with excess or stubborn fat deposits. Fat is usually taken from the lower back, flanks, and the transition to the buttocks, and sometimes from the inner thighs or around the knees.
With a single session, the breasts can typically be enlarged by up to one cup size. The limiting factor is the ability of the transferred fat cells to re-establish blood supply and survive. Fat cells receive nutrients only from their immediate surroundings, so good local blood flow is crucial.
If we overfill the tissues, the blood supply becomes compromised, and the fat cells cannot survive; the body will eventually break them down and remove them.
After harvesting, the fat cells are processed using a special technique and then precisely injected into the breasts. The way the fat is transferred is key to the survival of the grafted cells.
A thorough preoperative assessment and discussion of your wishes and realistic possibilities are essential when planning fat transfer breast augmentation.
The procedure is performed under general anaesthesia and takes approximately 1 to 2 hours.
After 8 to 12 hours of observation, the patient can go home.
Recovery after surgery is relatively quick. Sutures are removed after one week. For 4 to 6 weeks, we recommend wearing a sports bra and adjusting physical activities accordingly. During the first two weeks, we advise resting and sleeping on your back. In the liposuction areas, compression garments must be worn for 4 to 6 weeks.
After just a few days, most patients can resume their usual daily activities with minimal restrictions.
The tiny scars left by the procedure are almost invisible. The final shape of the breasts becomes apparent a few months after surgery, once the tissues have settled completely.
Who is the procedure suitable for?
Breasts add femininity, attractiveness, and sensuality to the body. As nature has not distributed volume equally, many women wish for slightly fuller breasts without a dramatic change in size.
Breast augmentation with your own fat is an excellent option when implants are not desired or not suitable, or when we aim for a subtle, natural-looking enhancement.
The procedure is also suitable for men who want a more sculpted, defined chest.
More about the procedure
Before breast augmentation with fat, we harvest your own fat using liposuction, which simultaneously reshapes the donor areas with excess or bothersome fat. Common donor sites include the lower back, flanks, and the transition to the buttocks, and sometimes the inner thighs and knees.
With a single treatment, it is usually possible to increase breast volume by up to one cup size. The limiting factor is the capacity of the transplanted fat cells to establish a blood supply and survive. Since fat cells are nourished only by the surrounding tissue, good blood flow in the recipient area is essential.
If we inject too large a volume, the tissue can become overly tense, blood vessels may be compressed, and the grafted cells will not receive enough nutrients and oxygen. In that case, the body breaks them down, and they disappear.
Once harvested, the fat is processed using a specific protocol and then carefully injected into the breasts in multiple small passes. This technique maximises the surface contact between fat cells and healthy, well-perfused tissue, which is crucial for long-term survival.
A careful preoperative consultation, examination, and clear communication about your expectations and what is realistically achievable are key to planning fat transfer to the breasts.

How much fat survives in the long term?
In the first few months after surgery, the body usually resorbs a portion of the transferred fat. On average, about 60 to 80% of the injected fat survives. After a few months, the final, stable result becomes visible – and is considered permanent.
However, the transplanted fat cells behave like your natural breast tissue and are subject to the same physiological changes caused by ageing, gravity, significant weight loss, pregnancy, and breastfeeding. For this reason, changes such as sagging, loss of volume and decreased fullness can reappear over time, just as they can in natural breasts.
If needed, the procedure can be repeated.
Can foreign (donor) fat be used?
No. Fat from another person is foreign tissue containing foreign antigens and antibodies. In life-saving organ transplantation (heart, kidneys, liver, lungs), powerful immunosuppressive drugs are required to prevent rejection.
For aesthetic procedures, such as reshaping the breasts or other body areas, the use of donor fat is neither safe nor acceptable – we can only use your own fat.
Fat transfer breast augmentation and liposuction
Fat is obtained by liposuction from areas where there is sufficient fat or where liposuction is performed primarily for body contouring. For this reason, the procedure is not suitable for individuals with very little body fat.
Because the survival of the transferred fat depends on nutrients from the surrounding tissue, we must be very careful not to inject excessive amounts at once. Overfilling would compress the tissues and blood vessels, reducing blood flow and leading to the death of the transplanted cells.
This is why, with fat alone, we can usually enlarge the breasts by up to one cup size per session. Fat transfer cannot create as dramatic an enhancement of the cleavage as implants can.
Fat grafting to the breast is therefore not suitable for patients seeking a striking, highly noticeable change. In those cases, we recommend classic breast augmentation with implants.
Fat injections cannot change the fundamental breast shape, cannot lift the breasts, and cannot significantly alter nipple position.
Scars are minimal, as fat is injected through tiny skin punctures. Only the minor entry points for fat harvesting and injection may be visible.
Is the result permanent?
In the first months after the procedure, the body breaks down a smaller portion of the grafted fat, with about 60 to 80% of the transferred volume usually preserved. After several months, the final result is considered stable and long-lasting.
Because the transplanted fat cells behave like natural breast tissue and are subject to the same physiological influences – aging, gravity, weight loss, pregnancy and breastfeeding – changes such as sagging or loss of volume can reappear over time.
If this happens and the patient wishes, the procedure can be repeated.
Am I a good candidate?
We harvest fat via liposuction from areas with adequate fat and where body contouring is also desired. Patients with very little body fat are therefore not ideal candidates for this procedure.
Because the survival of grafted fat depends on good blood supply and nutrients from the surrounding tissue, only a limited amount can safely be transferred at one time. Overfilling would compress the tissues and blood vessels, and the transplanted cells would not survive.
In practical terms, this means that with fat alone, we can usually increase the breast size by up to one cup size. Fat transfer cannot create as pronounced an upper pole or cleavage as an implant can.
Fat grafting is therefore not an ideal solution for patients seeking a dramatic, highly visible enlargement; in such cases, we recommend augmentation with implants.
With fat injections, we cannot significantly change the breast's structural shape, nor can we reposition the nipple or lift the breast.
What are the scars like after the procedure?
Scarring is minimal. Fat is injected through tiny needle punctures, and the entry sites for fat harvesting are also very small. The resulting marks are usually almost invisible once healed and are often hidden in natural skin folds.
